Patient Information Related to Coronavirus
We continue to monitor the outbreak of the Coronavirus (COVID-19). As a healthcare provider, we have an obligation to our patients, providers, staff, and community. The CDC and KDHE recommendations are updated regularly and therefore our practices will change as needed to remain in compliance with these agencies recommendations to monitor and manage the spread of the virus. The following processes will be implemented:
Screening and temperature checks prior to admittance to building for appointment or as a visitor to the building.
Have you had contact with anyone who is quarantined, is self-monitoring, or has been ill with a fever, cough, runny nose, or having trouble breathing due to possible exposure to COVID-19?
Have you traveled to Arizona in the last 14 days or have you attended a mass gathering event of 500 people or more?
International travel on or after July 14 to countries with a CDC Level 3 Travel Health Notice, including China, Iran, European Schengen area, United Kingdom, Republic of Ireland, or Brazil?
European Schengen countries: Austria, Belgium, the Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, the Netherlands, Norway, Poland, Portugal, Slovakia, Slovenia, Spain, Sweden and Switzerland.
Are you experiencing any respiratory symptoms that include any of the following?
- Dry Cough
- Chest Congestion
- Sinus Congestion
- Runny Nose
- Body Aches
- Shortness of Breath or difficulty breathing
- Sore Throat
- Loss of Taste or Smell
If you answer YES to any of these questions, please be advised that we will request you reschedule your appointment. 14 days is required before rescheduling your appointment.
We request you wear a MASK when in the clinic
- All visitors will be screened in the same manner as the patients
- Visitors will be limited to one person, preferably necessary to make medical decisions
- Visitors answering yes to exposure or respiratory symptoms will not be allowed in clinic
